Patient Forms
It is our office policy to only schedule new patients after we receive completed New Patient forms.
Please open and print (or download and print) the PSMC New Patient Packet (PDF) link below. It is important to be as precise and thorough as possible when completing the forms. Once completed, please send to our office along with a copy of your insurance card (if applicable). Once our staff receives your paperwork, you will be contacted to schedule your first appointment with Dr. Ximenes. We appreciate your consideration in accommodating this office policy.
New Patient paperwork can be submitted to our office by fax (512) 306-8425, email (newpatient@tpsmc.com) or regular mail.
Please note: Please bring the original New Patient forms with you to your first appointment (unless they are mailed ahead of time).
